COVID Preparedness
Effective June 3, 2020, the squash club has allowed me to have access to the front room of the club to resume physiotherapy treatments. As expected, there are many guidelines, from both the club and the college, to which we must adhere. These restrictions are to protect the health and safety of all staff and patients. I encourage you to read through the next few paragraphs to familiarize yourself with all procedures.
Please only come five minutes before your appointment to limit traffic in the facility. You will enter and exit through the front door; and be allowed to use the washroom only. We ask you to avoid the rest of the club.
If you have one, please bring a mask to your appointment. If you do not have a mask, one will be supplied for you. Please try to use your own personal mask as supplies are limited. You will be asked to wear it during your appointment.
Only patients are allowed in the club. If someone does accompany you, they will have to wait in the car or outside.
You will be asked COVID pre-screening questions on arrival. If you reply yes to any of these questions, you will be asked to return home. I would be happy to reschedule your appointment. A virtual appointment is also an option.
Each patient will be asked to complete Release of Liability, Waiver of all Possible Claims and Assumption of Risk form. You can contact me for copy or there will be a copy available at your treatment.
Scheduling has changed a little to allow for increased sanitization between patients.
If you do not feel comfortable with these circumstances, or no longer need this appointment, please let me know. I would be happy to keep you on a list for when these precautions are lifted. Having said that, I expect that some type of these precautions will be in use until there is a vaccine.
